These forms will not work for everyone. Answer these questions to see if they might work for you.

  • Do you want to establish parentage for more than one child?
  • Do you want genetic testing?
  • Do you need to include more than two possible parents?

If you answered YES to any of these questions, these forms will NOT work for you.

You can contact an attorney or the Department for Children and Families Child Support Services to help you establish parentage/paternity.

If you can't pay the filing fee, complete and submit a Poverty Affidavit. The judge will decide whether you will have to pay the filing fee.
