Are you starting (filing) a parentage case or responding to a parentage case?

 


These forms will not work for everyone. Answer these questions to see if they might work for you.

  • Do you want to establish parentage for more than one child?
  • Do you want genetic testing?
  • Do you need to include more than two possible parents?

If you answered YES to any of these questions, these forms will NOT work for you.

You can contact an attorney or the Department for Children and Families Child Support Services to help you establish parentage/paternity.
