Notice of Updated Power of Attorney & CINC Forms
The Kansas Judicial Council has created and amended the following legal forms.
Forms are available for download under Legal Forms.
Power of Attorney – 1 new and 1 amended form
The General Durable Power of Attorney was updated to clarify that it does not apply to healthcare decisions. Healthcare related decisions are made by the person authorized by the Durable Power of Attorney for Healthcare Decisions.

Child In Need of Care – 1 amended form
Due to user feedback, the sentence directing how a motion for additional time may be requested was removed in paragraph 8(b). This form is NOT a required form under S.C. Rule 174.
