Notice of New and Amended Guardianship and Conservatorship Forms
The Kansas Judicial Council has created and amended the following legal forms.
Forms are available for download under Guardianships, Conservatorships, Other Protective Arrangements.
Guardianship and Conservatorship Forms - 19 New and 4 Amended
These forms have all been created or amended due to Kansas' new guardianship and conservatorship laws that take effect January 1, 2026. Additional guardianship and conservatorship forms are being drafted and will be published in 2026 as they are completed.

- This one document replaces the Basic Instructions for Conservators, Basic Instructions for Guardians, and Basic Instructions for Guardians and Conservators.
- Annual Accounting
- This replaces the Annual or Final Accounting.
- Conservatorship Inventory
- This replaces the Inventory and Valuation Form.
- Report of the Guardian of the Child or Adult Subject to Guardianship
- This replaces the Guardian's Annual or Final Report on Condition of Ward.
Previous Versions of Forms
The previous version of the amended forms will be available through February 28, 2026. You should only use the previous versions of these forms if your judge specifically requests you do so. The previous versions of the forms are available here.
